Reporting frequently focuses on activity rather than outcomes, masking emerging risk. This erodes confidence at senior level and increases the likelihood of late intervention, cost escalation and delivery failure.

Strategic initiatives commonly cut across teams, partners and suppliers, each with competing priorities. Without clear governance structures and decision rights, accountability becomes blurred and escalation slows. Decisions are deferred, issues circulate without resolution and delivery teams operate in silos. Effective governance is essential to enable timely decisions, not add bureaucracy.

Delivery risk increases sharply during mobilisation, recovery and cutover phases. Where organisations lack experienced leadership or PMO capability at these points, progress stalls and uncertainty grows. Internal teams may be stretched or lack experience of comparable complexity, increasing the risk of delay and disruption.
